Hazardous occurrence investigation and reporting are a vital part of every work place health and safety program. In conducting an effective investigation, attention is paid to the roles and responsibilities of everyone involved. The methods of notification, investigation, analysis and reporting of major and minor hazardous occurrences must lead to corrective actions that prevent or reduce the number of hazardous occurrences. This publication examines these topics to help those who share responsibility for the investigation of hazardous occurrences.
